Congregant Service Selection
Submit your Service Selection to receive your High Holy Day name badge(s). Each member will receive one name badge, which provides access to all High Holy Day services. Adults attending Family and Tot Services may also request a name badge for those services. Requests must be made by August 14th, at which time the High Holy Day forms will be open to the community.

Associate and Out-of-State Congregant Service Selection
Associate and Out of State members are encouraged to purchase name badges for $180 or $300, depending on service selection.

Reciprocity
Chicago Sinai welcomes congregants from other URJ congregations through the reciprocal congregant agreement, subject to available capacity. Please submit a form to request your name badge(s).
